Qualification Requirements for Individual Producers

1. Your 2011 Conference of Champions production will be based upon your total fixed net paid annualized target new premium and total fixed net weighted annuity production from 1/1/10 - 12/31/10. Only proprietary fixed insurance products of LSW and National Life will count toward production requirements.
   
   
2. Producers with persistency of less than 65% may not receive an invitation. This persistency minimum does not apply to producers that have less than 10 cases or $10,000 of issued premium between 10/01/07 and 9/30/08.
   
   
3. Production credit for each policy will be given to the agent whose signature and number are shown on the application. Credits may not be transferred to another producer. We reserve the right to adjust premium weightings for case size, family or controlled business or any other unusual circumstances.
   
   
4. There is no cash alternative or substitution for the Conference of Champions.
   
   
5. You must be a producer actively writing with LSW, eligible to participate and in good standing to receive an invitation. LSW reserves the right to determine eligibility based on company records.
   
   
6. Sales of certain fixed life and annuity products, if prohibited by your broker/dealer, may not be eligible. Consult your broker/dealer for additional information. Representatives registered with broker/dealers affiliated with National Life Insurance Company may not count indexed annuity sales towards qualifications.
   
   
7. Awards may be taxable and IRS form 1099 will be issued if applicable.
